Application Process


New Clients & Renewals

New patients or renewals (provided you have the correct application) can use the following steps to obtain an appointment / medical marijuana card:

  • Step 1: Qualification Confirmation

Confirm you have a qualifying condition by either visiting thegreenscript.com, the Department of Health’s Website or by calling us at 667-7778.

  • Step 2: Medical Records

We will need your medical records for the past year. If you have several doctors the records from your primary care or the specialist for your condition should be adequate. Your doctor will need you to sign a form authorizing them to fax your medical records to us at 401-244-7133. We have a form that you can use on our website.

  • Step 3: Consultation Appointment

Once we have received your records you will be contacted to schedule an appointment. The fee for the consultation is $200, which includes our physician reviewing your records, performing a complete physical exam, completing your application and the second visit. The state requires semi-annual evaluations in order to have your license re-certified. We accept cash, check, credit card or money order.

The Department of Health will contact you when approved for a scheduled photo / identification card.

*Applications are not accepted in person. All applications must be mailed to the Department.

Requirements For Minors – (Under 18 Years of Age)

In addition to the requirements listed above, minor patients MUST designate a custodial parent or legal guardian as one of their primary caregivers. Additionally, a Minor Form must be completed, signed, and submitted along with the Patient Form.

We are not permitted to provide medical marijuana, and are prohibited from servicing patients in MA or CT. Please note, bipolar disorder, anxiety and insomnia are not qualifying conditions at this time.

Caregiver Requirements

Caregiver information is always provided by the patient. Caregivers must be 21 years of age to apply for a caregiver registration.

All caregivers will need a Background Check called a BCI. This can be done at the Attorney General’s Office:

Helpful Facts

The application process takes 4-6 weeks from the date it is accepted in the Department of Health’s (DOH) office. Incomplete applications will be returned, so double check! The DOH will not give information regarding application status over the phone due to confidentiality concerns.

If you have a medical marijuana card in Rhode Island that is expiring or already expired the DOH has a renewal form as your application. Email mmp@health.ri.gov to obtain the correct application.
